POSITION PURPOSE:

The Case Manager/Employment Specialist will be responsible for developing and monitoring the case plans of participants in the Re-entry Program. The incumbent duties require close coordination, collaboration, tracking, and monitoring of participants to ensure compliance with their service plans. Service areas will include Hertford, Gates, Chown, and Bertie Counties and will partner with agencies to ensure program participants are engaged and supported in services that include employment/training, educational attainment, and other supportive services.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  •        Promote the mission, vision, and values of the organization.
  •        Service a caseload of ongoing participants, all of which will be justice-involved individuals.
  •        Provide individual case management sessions with all clients at regularly scheduled intervals.
  •        Conducts initial screening to determine the level of service and support needed.
  •        Develop individual case plans for all clients that identify barriers to successful re-entry.
  •        Knowledge of the North Carolina Judicial System and/or community resources with demonstrated experience working with justice-involved youth.
  •        Document all clients' contact and progress, including time and date, type of contact, outcome, and plan of action.
  •        Other duties as assigned.

 

QUALIFICATIONS

Education/Experience: associate or bachelor’s degree in a preferred related Social/Behavioral Science field. 1-2 years' experience in delivering case management services.

 

SKILLS/ABILITIES:

  • Good written and verbal communication skills; critical thinking, problem solving.
  • Ability to work effectively with people of diverse educational and cultural backgrounds, from justice-involved to public officials.
  • Computer literacy, including proficiency with MS Office applications and database use. Ability to draft reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers, clients, and the public.
  • Ability to travel within the city and surrounding communities. Select national travel may be possible.
  • Knowledge of the Criminal Justice system and/or experience with justice involved is preferred.

 

HIRING RANGE: $45,000-$48,000 annually
